CLOSED TO NEW REGISTRATIONS


With effect from Monday 1 August 2016 the surgery will be not be accepting any new registrations from patients moving into the area or wishing to transfer from another local surgery.


 We will still be accepting:


* New babies born to mums registered at the practice


* Students coming back from University to live with parents registered with the practice


* Temporary patients, who are staying for less than 2 weeks, with patients registered at the practice


This will last until a new partner joins the practice or for 12 months


Sadly our circumstances remain unchanged and an application is in place to extend the list closure for a further year



FLU VACCINATIONS



The Surgery is holding two flu vaccination 'drop in' clinics this year. They will run from 0830 - 1230 on the following dates.



Saturday 23rd September for patients aged 65 and over



Saturday 21st October for all other eligible patients including children.


Eligible = those patients in the 'high risk' groups detailed below.


Please note we will not have any childrens vaccinations available prior to this date



Both Flu clinics will be scheduled to run by surname order so please try to attend at the relevant time wherever possible. You don't need to book, just turn up.



A - C    0830 - 0900


D - F    0900 - 0930


G - I    0930 - 1000


J - L     1000 - 1030


M - O   1030 - 1100


P - R    1100 - 1130


S - U    1130 - 1200


V - Z    1200 - 1230




If you do not wish to have the flu vaccination this year please let us know so we can update your medical records.


 


You are Eligible for a flu vaccination if you are in one or more of the following high risk groups;


 


Those over 65 years of age and


Carers


Pregnant


Patients with any of the following: 


    Heart Disease


    Diabetes


    Suffered a stroke


    Significant Kidney or Liver Disease


    Chronic Bronchitis                                    


    Emphysema


    Significant Asthma


    Patients on Immune suppressing Drug


    E.g. Steroid tablets,methotrexate or Azathioprine.


    Patients with a BMI of over 40.

Confidentiality & Medical Records

Locked blue folderThe practice complies with data protection and access to medical records legislation. Identifiable information about you will be shared with others in the following circumstances:

  • To provide further medical treatment for you e.g. from district nurses and hospital services.
  • To help you get other services e.g. from the social work department. This requires your consent.
  • When we have a duty to others e.g. in child protection cases anonymised patient information will also be used at local and national level to help the Health Board and Government plan services e.g. for diabetic care.

If you do not wish anonymous information about you to be used in such a way, please let us know.

Reception and administration staff require access to your medical records in order to do their jobs. These members of staff are bound by the same rules of confidentiality as the medical staff.

Freedom of Information

Information about the General Practioners and the practice required for disclosure under this act can be made available to the public. All requests for such information should be made to the practice manager.

Access to Records

In accordance with the Data Protection Act 1998 and Access to Health Records Act, patients may request to see their medical records. Such requests should be made through the practice manager and may be subject to an administration charge. No information will be released without the patient consent unless we are legally obliged to do so.

Customer service form Suggestions And Complaints

We are always keen to hear of any ideas to further improve the services we offer. Please feel free to talk to any of the team in confidence or to send your comments in writing.
Should a problem arise, our practice staff will ensure that you are taken to a private area so you may discuss your concerns in complete confidence. Most problems can be sorted out quickly and informally in this way. If you wish to make a complaint, we would like you to let us know as soon after the incident as possible. Your complaint will be fully investigated and you will receive a response within ten working days. A leaflet explaining the surgery complaints procedure can be obtained from any member of staff.

Violence Policy

We aim to treat our patients courteously at all times and expect our patients to treat our staff in a similarly respectful way. We take seriously any threatening, abusive or violent behaviour against any of our staff or patients. If a patient is violent or abusive, they will be warned to stop their behaviour. If they persist, we may exercise our right to take action to have them removed, immediately if necessary, from our list of patients.
