Flu Clinics 2019

Apologies for the late flu notification of this years flu clinics.  Unfortunately, our flu vaccines have been delayed again this year.  The vaccine for the under 18's is yet to be released for us to order so we have made the decision to push ahead with the clinics even though this does mean we will have to hold a separate one for the Under 18's at Rainbow surgery.  We are confident we will have the vaccines in time for the Doddington and Manea clinics.

If we have a mobile telephone number for you we will send/or already have sent an email message advising of the clinic we would prefer you to attend.  If the suggested time or date is not convenient please attend one of the other clinics below.  With a shortage of nurse and Health Care Assistant clinics during the week it would be a real help if you could attend on one of the designated Saturdays (We are currently recruiting for more staff).

The clinic dates for 2019 are as follows:

ANNUAL WELLBEING REVIEWS

You will now be invited for your yearly annual wellbeing reviews in your month of birth. The review will be different for each patient depending on what pre-existing conditions are present and it may require more than one appointment. 

These conditions are as follows:

BP + Pulse:    Hypertension, Atrial Fibrillation
Cardiovascular:Coronary Heart Disease, Chronic Heart Failure, Peripheral Arterial Disease, Stroke
Diabetes:Diabetes.  Pre-Diabetes, Gestational Diabetes
Renal:Chronic Kidney Failure
Mental Health:Bipolar, Dementia, Learning Disability, Psychosis
Respiratory:Asthma (if bronchodilator issued in last year), Chronic Obstructive Pulmonary Disease
Other:Rheumatoid Arthritis, Coeliac Disease, Severe Frailty, Post-Bariatric Surgery

To book the appointment(s) please:

  • Phone Reception at your Surgery
  • Please can you also ensure that your contact details are up to date along with consent for us to contact you on them e.g. mobile number, email address.

In the future we will no longer be sending letters.

 

We may need to do blood tests at this appointment.

 

It is important to attend your review appointment so we can ensure your condition is properly controlled and managed. If you run out of your repeat medication and have not had a review, we will not be able to re-authorise your repeat medication.

TELEPHONE UPDATE  

The phone system across Fenland Group Practice has been replaced which will now allow for all sites to take calls  for appointment bookings from 0800. Calls are answered in order across the sites.

We have been looking at how we can improve access to Practice services; DoctorLink is our new online service where you can access advice and appointments on line.

For all urgent appointment, we ask that you undertake a symptom assessment through DoctorLink

The symptom checker will ask you a number of questions to help to identify your problem.

The outcome of your assessment will direct you to the right treatment, whether emergency care, your GP, nurse or self care.

If your outcome indicates that an appointment is required, you simply click on the request appointment link.

Once the request has been submitted, you will then be contacted by the Practice Team, to arrange an appointment within the identified timeframe (as outlined in the outcome of your symptom checker).

 The information you provide will be shared with our clinical team (where appropriate) and will assist in providing robust information of your problem to the required clinical team member prior to your appointment.

GDPR

On 25 May 2018, the General Data Protection Regulation (GDPR)  will be enforced across Europe, including the UK. The law exists to give people better control over what organisations can do with their data. To comply with this legislation FGP has produced a Privacy Notice which tells you how we use the information we hold about you. You will find this Privacy Notice in the GDPR section of the website where we will also be adding further updates and policies.

New Prescription Services

Over the next few months we will be changing the way we handle repeat prescription requests, and it will be EASIER for you.  Please see the prescriptions page for more details.

OVER THE COUNTER MEDICATIONS

** We are being encouraged to ask you to buy certain cheaper medicines and creams over the counter e.g. antihistamine, painkillers (including gels), antacids, decongestants, cough and cold treatments**

REGISTER FOR ON LINE ACCESS TO YOUR RECORDS

This can enable you to book appointments, order repeat medications, access blood test results and allow further access to your records.

You can download an application form from the website or collect a form from Reception.

NEXT OF KIN DETAILS

We encourage all patients to provide us with next of kin details which we can record in your medical notes. Please advise Reception of the names and contact details where possible or email them to no-reply.fenlandgp@nhs.net
